Service Coordination
Service coordination is a way that helps bring services to children and families in a simple and organized plan. Families are part of the planning to organize all the services a family needs. All the organizations and agencies a family needs meet at one time to make a written plan.
Four Stages of Service Coordination:
- 1. Referral to Holmes County FCFC; Assessment of needs; and Identification of the team members that can help.
- 2. Initial Team Meeting in which the team identifies needs of the family, coordinates existing services and supports, and matches resources with needs. A written plan is developed.
- 3. Plan implementation/Reviews occur at least every ninety days. Progress toward goals is reviewed and adjustments are made as needed.
- 4.
Transition plans are made as the team nears goal completion.
